Eligibility
Mobility Management Plan (MMP):
-Reside in Santa Clara County
-Complete a new Senior Nutrition Program(SNP) registration form
-Qualify as low-income based on the Elder Economic Security Index(EESI) for Santa Clara County
-Need the transportation benefit to be able to attend the meal program
-Bus passes and gas cards require regular dining site attendance
-Paratransit participants must be certified paratransit eligible by the Santa Clara Valley Transportation Authority
RYDE:
-Must reside in Santa Clara County
-Must be 65 years of age or older, and ambulatory (able to walk on their own, or be able to walk assisted with a cane or walker).
-Participants are allowed only one companion per passenger

To request a ride, call 2 business days in advance. Rides can be booked up to 4 weeks ahead.
