Location Overview

Provides seniors with the care and resources needed to age safely at home. Serves as an alternative to nursing homes and helps seniors maintain their autonomy.

Program services encompass both home care and transportation to clinics and appointments. Appointments include medical care, dentistry, occupational therpay, and physical therapy. Home-care services include the following:
  • Escort assistance to medical appointments, as needed
  • Transportion to and from PACE center visits or other health care appointments
  • Personal care and medication reminders
  • Laundry and light housekeeping
  • Light meals preparation
  • Feeding
